Population health is an approach that aims to reduce health inequalities across an entire population, promote health and wellbeing, and improve both physical and mental health outcomes.
This is a priority area of focus for the NHS Confederation. We support our members to tackle health inequalities as part of a population health management approach.
We aim to equip our members with the tools and insights they need to address disparities in health across their communities.
