Primary Care Federation Network
In April 2022, we launched our new enhanced network for primary care – the only network bringing together primary care with the rest of the healthcare system.
As a result, our primary care networks – the PCN Network and Primary Care Federation Network – united, resulting in a new and powerful voice for the sector. Find out more.
The Primary Care Federation Network offers a unified voice for primary care at place. We develop population-based strategies, coordinate service delivery, provide governance and assurance, and share innovation and best practice.
We support federations to work collaboratively with primary care networks and other local partners across health, social and community care to provide better integrated, innovative and responsive care for patients, reduce health inequalities and improve the health of the local population.
Our network is defined by members that hold the following values:
- local – put the local population at the heart of what we do and helping to identify the needs of communities in a ‘place’
- integrated – support an integrated approach with local NHS and care providers to co-produce and deliver care
- of the NHS - deliver NHS services through NHS contracts for NHS patients
- driven by a social purpose – reinvest any surplus in our communities.
