Join us for a joint session with NHS England and Burnley East Primary Care Network to explore the impact of proactive patient engagement. The Burnley East team support a diverse population which includes patients from traditionally underserved communities.
To improve health outcomes the team have led an active campaign of engagement, working directly with patients outside of traditional healthcare settings and taking services to community settings that are more familiar and comfortable for patients, through various outreach projects in order to breakdown barriers and share the full range of health services available. From classes to huge Mela gatherings, the PCN team are supporting patients to take ownership of their health and care.
