  • Right now, one in four people in the UK dies without the care and support they need. Our new report shows that by 2048, the number of people with palliative care needs will climb from 147,000 to over 730,000. If current trends continue, this means hundreds of thousands of people will miss out on essential care and support.

Marie Curie has five key evidence-based commitments to close the gap in end-of-life care:

    • More coordinated single point of access hubs – through working in partnership with healthcare providers, to reduce the complexity of referral pathways for people affected by terminal illness.
    • Working with the NHS, healthcare providers, and people affected by terminal illness – to co-create and deliver end of life services which are coordinated, cost-effective, and keep people's unique needs at their heart.
    • Improve equity of access – through more targeted support, particularly those living in areas of deprivation or underserved groups.

  • The 8am rush is a chronic problem faced by primary care practices.  Smart Triage helps NHS GP practices and Primary Care Networks (PCNs) solve this issue by giving patients the power of self-service.  From assessing symptoms to facilitating self-booking appointments, Smart Triage safely navigates patients to the most appropriate care setting, without input from practice staff.

Smart Triage is used by those PCNs and GP practices who want to outperform the new Modern General Practice Access (MGPA) model.

Customers use it to automatically triage incoming requests, without the need for practice staff. This improves upon the MGPA model, which relies upon human navigators to triage all requests.  By liberating both administrators and clinicians from the triage process, the speed, safety and accuracy of patient triage can be combined to improve the delivery of care.
