During this session, you will hear from colleagues from Wide Way Medical Centre and London Ambulance Service who have co-designed and co-delivered a joint call navigation offer to address the 8am rush in general practice. This novel approach has brought to life the Fuller vision of integrating primary and urgent care to deliver an access model that makes sense to patients and makes the best use of available services on offer.

Topics covered include:

  • Delivering transformational change across organisational and cultural boundaries, and in short timescales
  • Building consensus around approach and resource allocation
  • Digital interoperability
  • Population health management
  • Empowering patients through education and information.

Chair: Matt Inada-Kim, consultant in acute and general medicine and visiting professor at Hampshire Hospitals and University of Southampton

Speakers include: Dr Johra Alam, Clinical Director IUEC for London Ambulance Service NHS Trust and Dr Sy Ganesaratnam, GP Clinical Director for East Merton PCN
