Reshaping primary care

We're working with the National Association of Primary Care (NAPC) to deliver the pioneering Primary Care Home Programme.

The programme aims to strengthen and redesign primary care around the health and social needs of local communities. 

It's based on the Primary Care Home (PCH) model, a form of multispecialty community provider, and aligned with the vision of the Five Year Forward View.

Key features include:  

  • provision of care to a defined, registered population of between 30,000 and 50,000
  • aligned clinical financial drivers through a unified, capitated budget with appropriate shared risks and rewards 
  • an integrated workforce, with a strong focus on partnerships spanning primary, secondary and social care
  • a combined focus on personalisation of care with improvements in population health outcomes.

Fifteen 'rapid test sites' across the country are piloting this new, enhanced approach to primary care.
