A lack of collaboration between local primary care providers meant that out-of-hospital care for patients was not very well coordinated.
Larwood and Bawtry Primary Care Home has identified an opportunity to develop a genuinely integrated team for out-of hospital-care. Staff have been developing close working relationships with community services, nursing homes, the voluntary sector, mental health colleagues and local secondary care services.
The team are able to develop a more targeted approach for those people that use primary and secondary care services the most, working with them to identify their needs, address any gaps in their care and deliver care through a coordinated team. This is expected to improve patient outcomes as well as efficiency.
