Local health and care services in East Grinstead are often more reactive than preventative and the majority of care is delivered in hospital rather than in the community.
This means that care is not always being delivered at the right time and in the best place for the patient. It also means resources are not being used as effectively as they could be, putting pressure on costs and sustainability of services.
The East Grinstead Partnership is developing more proactive community-based services, helping to keep people well, as well as providing care closer to home if they become ill. Multi-disciplinary clinical teams are coordinating this care with a focus on early intervention and living well at home.
