Royal Pharmaceutical Society launches guidance on safe transfer of care

The Royal Pharmaceutical Society (RPS) has produced two guides, one for healthcare professions and one for commissioners, on how to ensure the safe transfer of information about medicines.
 

Safe systems

pharmaceutical boxesThe guide for healthcare professionals outlines their core principles and responsibilities, while the guide for providers and commissioners of NHS services provides valuable and practical information about how to ensure there are safe systems in place.

Medicines Use Review

The RPS guidance complements the introduction of targeted Medicines Use Review (MURs) in the Community Pharmacy Contractual Framework, with one of the three target groups being patients recently discharged from hospital who have had a change to their medicines.

NHS Employers and the Pharmaceutical Services Negotiating Committee (PSNC) were part of a working group that helped to develop the guidance.
